Homebound status makes going to the doctor a burden that most prefer to avoid if possible. Illustrated herein is your understanding of the comprehensive nature of Florida Assisted Living Coalition.
We combine the technology of modern medicine with an old fashioned idea "doctor house calls" that resonates with our public. Care, conveniences, privacy and integrity are a few of the reasons why our physician network is so popular in Florida! We call this division of our organization Doctors to your Door a subdivision of Florida Assisted Living Coalition.
Our network of caring, licensed physicians, psychiatrists, practitioners, nurses and certified medical assistants will provide expert on-site diagnosis, triage care, and treatment at a fraction of the cost of an emergency room visit to homebound patients in congregate care independent, adult family group home and assisted living facilities. Thus making it a win/win for the insurance industry and the patient. The network operates in (4) counties: Miami, Broward, Palm Beach and Martin yet has the potential to reach other major cities in Florida. Our people first approach to healthcare allows patients, to avoid undue emergency room or hospitalization visits. The medical team can be ones primary care physician or a medical triage on behalf of one's PCP, specialist, or their caregiver. We bring doctors to your door!
Your network capabilities may cover a wide range of treatments from caring for chronic conditions to providing medication and monitoring of flu symptoms, hydration to food poisoning. Equally important, those providers improve quality of life for patients, while reducing overall costs for the health care industry. Physicians and medical technicians will deliver these services on call 24/7, 365 days a year managing minor to complex medical needs.
Providers work with a group of independent individually licensed physicians and by no means are a representative of (FALC) Florida Assisted Living Coalition. We help you to fill the gap in your care continuum. We are connecting a network not "recommending" a provider. Selecting a physician is a choice made by the patient or the patient's representative and should be an informed decision. Our mission is to introduce our publice to a physician in their area who will develop a relationship between the parties doctor/patient. Our relationship may encompass scheduling one's medical appointment. Information between the patient and their physician falls outside of Doctors to your Door auspices. Patient confidentiality is of utmost priority.

It is intended only for the use of the individual or entity named between the two parties unless specified otherwise. Any and all information is considered protected Health Information. Any dissemination, distribution or copying of patient information outside of patient profile "intake" is strictly prohibited.
